On February 19 - 21, in Barnaul, the Positive Development Non-Governmental Organisation of the Altai Kray arranged a training event to provide basic knowledge about HIV/AIDS, under the project aimed at developing non-medical services for PLWHA in the Altai Kray and funded by the Russian Health Care Foundation under the Global Fund Program: Promoting a Strategic Response to HIV/AIDS – Treatment and Care for Vulnerable Populations.

The training is expected to contribute to the development of HIV/ADS basic knowledge and skills to provide counselling support to PLWHA and their relatives.

The training was delivered to crisis and social/psychological counselling specialists from areas affected with the HIV epidemic (Slavgorod, Biysk, Zarinsk, Barnaul, Pospelikha Rayon, and Germans’ Rayon).

These specialists are keen to acquire HIV/AIDS counselling knowledge and skills because they are facing a really tangible need in such services and in cooperation with the Positive Development NGO to address provision of social and psychological support to PLWHA.

On February 16, 2008, a meeting of young people took place in the Maximum Club in Rostov-on-the-Don. Fifty seven young people gathered to celebrate St. Valentine Day. They enjoyed an animated entertainment program with a good anchor, music and a very relevant theme for young people. In breaks between competitions and performances, a short lecture was delivered to tell the audience about the STI and HIV transmission ways, HIV/AIDS video spots were demonstrated and a Positive Life Brochure was disseminated.

The event was organized by the Choice NGO and the Sparta Foundation (providing assistance with drug users’ rehabilitation) which implements a project to develop direct non-medical services to PLWHA in the Rostov Oblast (funded by the Russian Health Care Foundation). Such cooperation became possible owing to joint efforts of Rostov civil society organisations dealing with provision of non-medical and HIV prevention services.

According to Marina Akulova, Chairperson of the Irkutsk Oblast Department of the Russian Red Cross, the process of moving the Irkutsk Red Cross from its building was suspended.

She notes that Alexander Tishanin, Governor of the Irkutsk Oblast, gave an instruction to the Regional Department of Property Relations to clarify and settle the situation. Per notification from the Federal Agency of Federal Assets Management, the charity organisation had to leave the building by February 18, 2008.

Efforts to move the Red Cross from the building have been persisting since 2003. The Red Cross has been occupying this building for over 50 years. M. Akulova explaines that in 1972, the City Health Administration passed the building on the balance sheet of the Red Cross, but the property right remained with the Territorial Department under the Federal Agency of Federal Assets Management. She notes that last year, this Department demanded that the Red Cross should pay a rent in the amount of RUB 622,000 per month, and afterwards, this amount was reduced to RUB 150,000 by judicial ruling. However, currently, the payment for the premises is RUB 210,000 which is unaffordable for the organisation.

The Regional Red Cross has repeatedly requested to the Irkutsk Oblast Administration to provide premises for it, but no suitable buildings were found. Marina Akuliva says that the Red Cross prepared all needed documents to pass the building into its long-term gratuitous use. In her opinion, if they have to rent the building, increasingly growing rent payments will result into similar situations again.

Last year, the Irkutsk Regional Red Cross implemented 17 charity programs, and there are 20 on-going programs for this year. It provides charity support for various categories of people in need. The building homes the Stairs HIV Information and Consultation Centre, Seven-Coloured Flower Children Centre, a family health centre, and a social bureau. Psychological, legal, social and humanitarian care is provided here free of charge.

The Irkutsk Red Cross implements four projects funded by the Russian Health Care Foundation, which are aimed at (i) Providing access to health services and information for CSWs through outreach activities; (ii) Providing palliative care to PLWHA; (iii) providing psychological and social care to HIV-infected children with a focus on orphans; and (iv) operating a reference centre to provide support to children born of HIV-infected mothers, including HIV/AIDS prevention, treatment and social support.

Are the measures against HIV effective in the Irkutsk Oblast?

This question was in the focus of a meeting in the Regional Administration today. The epidemic is persisting. As earlier, the region ranks the first in Russia in the number of HIV-infected people. What programs are implemented there and what programs are needed? This issue was the first in the agenda.

In each block of apartments, there is at least one HIV-infected resident. This fact is referred to by specialists so that any one could grasp the scale of the epidemic in the Irkutsk Oblast. So far, nobody knows how to arrest the spread of the disease. Today, the HIV problem was on top of the agenda for the weekly reporting meeting of the Governor. The Deputy Director of the Regional Health Department presented an HIV response evaluation report. “The spread of the epidemic into the general population can lead to its uncontrolled growth. It should be noted that the level of welfare also affects the epidemiological situation. Increasing earnings of the region’s population make it more attractive for drug dealers”, said Tatyana Boiko, Deputy Director of the Irkutsk Oblast Health Department.

The report described the trends in the epidemic development since 2000, including its rises and falls and their causes. But such a detailed report did not satisfy Yuri Paranichev, Deputy Governor: “You should have confined only to the effectiveness. But it is the effectiveness which is missing in both the report and the preceding information. In the conclusions, the same facts are referred to, often repeatedly” said Yuri Paranichev, First Deputy of the Irkutsk Oblast Governor.

It was recommended to revise the report taking onto account all the comments. Pavel Tsikolin, Governor’s HIV Advisor, put forward a proposal to integrate civil society activities with the Regional Anti-HIV/AIDS Program. He explained that non-commercial organisations of the Irkutsk Oblast receive about US$ 30 million a year from various international sources. International best practices are already implemented in the region, but not everywhere. Close cooperation with the non-governmental sector will enable the regional authorities to implement the best programs where they have not been implemented yet. “Such an approach would help to attract additional resources to remote areas. That means that now most of activities are implemented in Irkutsk and Angarsk, with the rest of areas being not covered with such resources”, said Pavel Tsikolin, Governor’s Advisor.

Regional officials decided to consider this proposal separately and in a really participatory manner, involving all stakeholders: regional government (the Governor), civil society and representatives from international organisations providing resources.

On February 4 - 9, 2008, the Positive Development Non-Governmental Organisation of the Altai Kray is delivering training in non-medical services for participants of the Program: Promoting a Strategic Response to HIV/AIDS – Treatment and Care for Vulnerable Populations. The event is held in Barnaul.

The audience also includes representatives from a number of regions which joined the Program earlier.

During the six days, training participants will learn about:

• non-medical services, their role in treatment, prevention and mitigation of negative consequences of the AIDS epidemic;
• PLWHA’s needs in support and care: impact of HIV infection on life quality, needs assessment methods;
• involvement of HIV-infected people in programs of non-medical services: involvement principles and methods;
• arrangements and methods to provide social and psychological support to people with HIV and their relatives: objectives, advantages and constraints;
• basics of social project development: stages and principles of non-medical service project development ;
• basics of project management; human resource management, monitoring and evaluation, search for resources, organisation and delivery specifics of various activities, common rules for program progress and financial reports;
• basics of partnering with other organisations.

Each participant of the training will receive a folder with information and guidelines on all the areas covered during the training, and a list of useful internet resources.

Upon training, its participants are expected to acquire knowledge about key aspects of establishing and developing non-medical services for HIV-infected people, basic skills of social project development and management, and printed information and guidelines which would help them to develop non-medical service projects in their regions.

Currently, the Russian Health Care Foundation supports 27 on-going non-medical service projects in 12 Russian regions.

On January 21 - 26, 2008, the St-Petersburg Medical Academy of Post-Diploma Education delivered a course of training under the Program: Promoting a Strategic Response to HIV/AIDS – Treatment and Care for Vulnerable Populations (for program participants of year 3).

It was a six day event for training in initiation of antiretroviral treatment of adults and adolescents. The training covered 18 trainers from higher medical schools and regional AIDS Centres of the Republic of Osetia-Alania, Primorsky Kray, Khanty-Mansiysk Autonomous District, Ivanovo, Tyumen and Ryazan Oblasts.

The training was focused on implementation of HIV/AIDS training programs in higher and post-diploma schools for purposes of human resource capacity building to ensure quality treatment and care to people living with HIV/AIDS (PLWHA). Trained trainers would be able to transfer the acquired knowledge in HIV/AIDS treatment and prevention, to train medical personnel and trainers from health facilities in other regions. All the participants will receive needed information products prepared by the training organisers.

January 24, 2008 is the closing day of the first National Conference dedicated to Civil Society’s Role in HIV/AIDS Response in the Russian Federation organized by the Russian Harm Reduction Network, the PLWH community and the Forum of NCOs engaged in HIV/AIDS control in Russia, to discuss successful experience, problems and prospects in this area. The event was prepared with financial inputs from the Russian Health Care Foundation.

Participating civil society representatives and invited guests of the Conference discussed the HIV/AIDS situation in Russia, mechanisms to ensure civil society involvement into the development and implementation of the HIV/AIDS national strategy, and will word the position of civil society with respect to national AIDS programs. To date, it is critically important to discuss the most critical issues and prospects for civil society development in Russia, as well as such issues as its cooperation with business, public health institutions and decision-makers.

The Conference took three days, and each of them had a specific focus. January 22, the first day of the Conference, was entirely dedicated to the progress in achieving the goal of universal access to HIV prevention, treatment, care and support, barriers for civil society, the PLWHA community and public sector entities on their way to ensuring universal access; respective plans and prospects.

Yesterday, on the second day of the Conference, its participants discussed cooperation between government and civil society towards improving the effectiveness of the response to the HIV epidemic in Russia.

Today, they will discuss such issues as treatment adherence, public awareness campaigns in mass media, program follow-up options and preparation of grant applications.

The Conference will be concluded with the adoption of its Resolution to state jointly identified HIV/AIDS actions, common positions with respect to the most critical issues related to the need to attract public funding and to take into account civil society proposals on priority setting under the national HIV response strategy.
